Acne treatment has advanced so much in the last ten years. The modern acne treatment uses high-spec laser technology to destroy the acne bacteria and stimulate the immune system.

Current treatments for acne involve the use of antibiotics for long periods of up to 6 to 8 months or harsh drying lotions like benzoyl peroxide that bleach clothes and hair and need someone else to apply it to tricky areas. Resistance to the commonly used antibiotics is currently around 65% in some areas of the UK.

There are many interacting factors which result in acne. These have become better understood over the last 20 years and now acne treatment can be better aimed to improve individual acne.
 
Acne is a disease of the very specialised sebaceous follicles found on the face, neck, back and upper chest and buttocks. These glands produce excess oil or sebum and also they collect abnormal amounts of old dead cells from the walls of the cysts. This results in glands filled with oil and blocked by cells. These may then just turn into black or white heads or may get really inflamed and turn into big red spots. The skin has its own resident bacteria, P.Acnes, which thrives on the oily/dead cell mix.     
 
P.Acnes is rapidly developing antibiotic resistance with resistance rates of up to 65% in some parts of the UK, explaining frequent treatment failures.
 
Work by Dr Chu, Consultant Dermatologist and world expert on acne has established that P.Acnes is a pigment containing bacteria which can be killed by exposure to certain wavelengths of light. This explains why all of us acne sufferers get better temporarily during periods of sun exposure. It also explains why the N-Lite V3 Laser system improves the outcome so enormously.
 
Other acne treatments are tailored as far as possible to the cause. The amount of oil can be reduced by using specialised drugs like Roaccutane which is a hospital only prescription in the UK.   
Many of our patients have used anabolic steroids which directly promote the development of increasingly active sebaceous glands and produce more oil. Stopping the steroids obviously will improve matters. Testosterone and testosterone derivatives are changed into the much more potent dihydrotestosterone in sebaceous follicles. In males, tackling the production of testosterone itself would be problematic.
 
The collection of old dead cells in the glands and on the surface of the skin can be tackled using retinoids such as tretinoin, isotretinoin and tazarotene. These are prescription only items except in low dose formulations. Salicylic acid, contained in some washes and also in the Freedomhealth range of chemical peels, is also very effective at reducing the collections of dead cells and thus blackheads etc.
 
Topical antibiotics, sometimes mixed with benzoyl peroxide will reduce the amount of the bacteria on the skin and thus reduce inflammation. Unfortunately, benzoyl peroxide may sometimes cause its own inflammation and as discussed, bacterial resistance to anti-biotics is growing alarmingly.
